Terms and Conditions
Because of certain restrictions, Life Support International, Inc. (LSI) cannot offer all products to all customers. We reserve the right to refuse sale of any product on our site if it falls under this criteria. For domestic orders by phone, fax, mail or e-mail please include, or have ready, the following:
- Full Name.
- Exact billing address of credit card, and shipping address (If different). Phone number, Fax number, and E-mail address (Whichever apply).
- The equipment you wish to order. You must specify Part Numbers, Descriptions, Quantities, Sizes and Colors.
- Shipping Preferences: We ship UPS (United Parcel Service) and FedEx (Federal Express). We will ship UPS to all addresses, except P.O. Boxes, if a preference is not specified.
- We normally ship the equipment to our customers the least expensive way possible. (We can supply an estimated cost of shipping and insurance if requested). We will charge your credit card the full amount for items ordered and exact shipping once the order is ready to ship.
- Credit card information. We accept VISA, MC, Discover and American Express. We process payments on a secure socket layer (SSL) with VeriSign. (If you are mailing in an order you will need to enclose your credit card information, money order or certified check drawn on U.S. funds, including shipping and handling charges, for the equipment).
- We require a $150.00 minimum on US Domestic Orders.
International Orders
For International orders by phone, fax, mail or e-mail please include the following:
- Full Name.
- Exact billing address of credit card, and shipping address (If different). Phone number, Fax number, and E-mail address (Whichever apply).
- The equipment you wish to order. You must specify Part Numbers, Descriptions, Quantities, Sizes and Colors.
- Shipping Preferences: We ship International UPS (United Parcel Service), FedEx (Federal Express) or Airfreight. We normally ship the equipment to our customers the least expensive way possible. (We can supply you with an estimated cost of shipping and insurance if you request it).
- We cannot accept credit cards for International Orders. We can only accept Bank Wire Transfers. Please choose the “Print to Mail / Fax” option when placing your order (instead of Verisign) and see below for transfer details.
- Recipient is responsible for all duties and taxes that may apply.
- We require a $200.00 minimum on any International Orders.
Bank Transfer Information
Payment must be made in US dollars. All Bank Transfer Charges must be paid by the Applicant. You will need to include $25 for Bank Transfer fees. LSI’s Bank Transfer information is as follows:

Shipping Charges
Shipping, handling and insurance charges depend upon the weight and volume of the box being shipped, the value of your order and the final destination of shipment. Due to restricted delivery areas, excessive weight, or oversized packages that cannot be serviced by one carrier, we may be forced to use multiple carriers when necessary. We will select the most economical method of shipment.
- Domestic Orders: UPS (United Parcel Service), and FedEx (Federal Express) are the preferred method of shipping.
- International Orders: International UPS (United Parcel Service), FedEx (Federal Express) and Airfreight are used for shipping. Recipient is responsible for all duties and taxes that may apply.
- Next Day Air Shipments are only available via phone orders, to physical addresses in the continental U.S. These are NOT available for P.O. Box, APO/FPO or international addresses. You must verify that items are in stock for next day shipping option
We can only give approximate shipping charges via E-mail, phone, or fax. We do not know the exact amount until we box the equipment and weigh it at the time of shipping.
When you receive your shipment, please note the total amount shown on the invoice is the amount charged to your credit card. You are only charged for the items shipped, plus shipping and handling. Your credit card will NOT be charged for backordered items until the backorder is shipped. Additional Shipping charges will be added to backordered items.

HazMat Shipping Policies
To place a HazMat order, call LSI at 215-785-2870 or order online and we will process your order. If you would like, you can also fax or email your order to us. Please include contact information as we will need to get some added information regarding your order.
A minimum HazMat charge of $25.00 will be added in addition to the normal shipping rate. Both HazMat and non-HazMat items may be combined to save shipping costs.
As with all our products, we welcome any returns you may deem necessary. However, HazMat shipping charges cannot be refunded except in the case of defective items. If you do need to return a HazMat item, please call us as certain regulations apply.
LSI uses FedEx as its HazMat shipper.

Written inquiries are also invited, and will be answered as fully and promptly as possible. Please enclose a self-addressed stamped envelope when writing.

Return Policy
Unused equipment returns are accepted within 30 days of the date of purchase. However, no equipment may be returned to LSI without first contacting us. If LSI approves the return of any goods, it should be understood:
- There will be a minimum restocking charge of 25%.
- Smoke signals cannot be returned.
- Custom orders made to your specifications cannot be returned for exchange or credit.
Note: We reserve the right to either replace or repair the items, or refund your money. Our guarantee covers any defect in material or workmanship occurring under normal use within a reasonable amount of time. If we must return equipment to the manufacturer for repair, and there is a charge, customers will be billed at cost for the charge.
In the event that you need to return an item, please follow these directions:
- Contact LSI prior to returning any item.
- All returned merchandise must be in original condition along with packaging (example: warranty information, instructions, box, etc.)
- Include a copy of the packing slip along with detailed instructions for our return department (e.g. credit, refund, or exchange).
- A note with full explanation of return must accompany any returned item.

Please include funds for postage on exchanged items.
HazMat shipping charges CANNOT be refunded for returns or exchanges.
After 30 days, we strongly suggest that any defective merchandise be returned directly to the manufacturer stating the date of purchase and source of supply. By doing so, time and expense will be saved.
Items ordered together are not necessarily shipped or delivered on the same day. Wait a few days before contacting us if you are "missing a package."

Damaged or Missing Items
For packages damaged during shipping or for missing items (except for those on backorder), you cannot collect for breakage, damage, or shortages unless you keep the original shipping container.
No matter what the shipping method, if any package shows signs of external damage, please do not accept it. Refuse it and insist that the carrier tell you what procedure should be followed. NOTIFY US IMMEDIATELY! If there is concealed damage - notify us at 215-785-2870 and we will tell you what procedure to follow.

- Use only those products designed for its specified use.
- Do not exceed recommended or stated forces, speeds, or other factors regarding safe use of the equipment.
- Read and follow all warning labels, manuals, instructions, training or experience requirements and recommendations and all recognized use procedures.
- Check and calibrate all similar equipment before each use.
- Be extremely careful and cautious. Know and examine your equipment before each use. Review emergency procedures before each use.
- Warning: Any person using equipment sold by Life Support International, Inc. in any manner is personally responsible for learning proper techniques involved, and assumes all risks and accepts full and complete responsibility for any and all damages, including death, which may result from the use of any equipment sold by Life Support International, Inc. Tensile strengths may decrease depending upon use or storage conditions.
